Do not delay filing your claim
  If you believe that you are unable to work, even if you do not know for how long, I would encourage you to file your application. The process of obtaining Social Security disability benefits can be a lengthy one. Therefore, if you delay filing your claim and you are out of work for an extended period of time, you will regret not filing your claim earlier. Also, some benefits may expire if you do not file promptly.

How to File
  You can file your claim in person at your local Social Security office, or by telephone 1-800-772-1213 or online at the Social Security website.

  As part of your application you will complete a disability report and work history. This report needs to be carefully completed. It will become part of the evidence in your case and is crucial to your determination of disability. Review the disability evaluation steps if you have further questions.
